DCC/CSA Prescription Drug Benefit

DCC/CSA Prescription Drug Forms                                               

The DCC/CSA Welfare Fund provides Prescription Drug benefits to all active members and their eligible dependents, and those retired members who choose to participate in the Welfare Fund.

FOR ACTIVE MEMBERS: There are two parts to the drug benefit.  The retail program is designed to be used when medications are immediately needed or are being monitored to achieve the correct dosage.  There is a 20% co-payment for each prescription.

When filling a prescription at the local pharmacy, be sure to present your MEDCO card.

The mail service program is designed to used for medications needed to treat chronic conditions, such as high blood pressure or high cholesterol, which will be needed for an extended period of time.  Members pay a co-payment of $ 10.00 for a generic medication or $ 25.00 for a brand-name medication  for a 60-day supply of the medication.

FOR RETIRED MEMBERS UNDER 65 ONLY:  There are two parts to the drug benefit.  The retail program is designed to be used when medications are immediately needed or are being monitored to achieve the correct dosage.  There is a $ 50.00 deductible for each person, after which there is a 20% co-payment for each prescription.

When filling a prescription at the local pharmacy, be sure to present your MEDCO card.

There is a $ 1,500.00 per person annual maximum for the retiree drug benefit.
