members and their eligible dependents are provided an optical benefit once each year. If you use a participating optical center you receive an eye examination ( New York only ), single vision or bifocal glass or plastic lenses, and a wide choice of frames, including a selection of designer frames with no out-of-pocket expense. If you choose frames not included in the basic selection, or special lens treatments such as UV coatings or tinting, or progressive bifocals, you will be charged a surcharge for these features. If you do not use a participating optical center, you will be reimbursed by the Fund according to the schedule of allowances established by the Fund |
