The Fund has established a post-hospitalization Home health Care benefit for members and eligible dependents who require such services as certified by a physician.  Eligibility requires that the patient be hospitalized for at least three days and home care services commence within 30 days of discharge.  The benefit pays 80% of the covered charges to a maximum of $ 6,000 per year. There is a lifetime maximum of $ 18,000 for this benefit.  Members are encouraged to review the specific conditions for this benefit in their Benefit Description Book
