The CSA Welfare Fund provides a choice two dental plans to its
members. Unless otherwise requested, members are enrolled in the SIDS
Dental Plan.
 | SIDS Dental Program-Allows you to use any dentist. There is a
$ 25.00 per person annual deductible. If you use a SIDS
participating dentist, the only additional cost is for major work, where
there is a $ 50.00 co-pay. |
| Healthplex HMO - New York and New Jersey This is a prepaid
program, similar to an HMO. You must select a Healthplex
primary dentist, and may only use another dentist if referred by your
primary dentist. There is no out-of-pocket expense to you with the
Healthplex HMO plan |
